Responsibilities

The Nurse Practitioner is responsible for the day to day coordination and oversight of all aspects within the clinical management of patients. The Nurse Practitioner functions independently, while also collaborating with the clinical team. Thorough understanding of managed care delivery systems and utilization management is essential. Job duties include, but are not limited to:

  1. Assessment of patients to identify needs for services and support such as: DME and supplies, acute medical services, behavioral health services, and primary or specialist care.
  2. Assist patient and their families in identifying goals of care, family/support system, environmental, cultural and linguistic needs for the patient care plan.
  3. Complete appropriate assessments within contractual timelines.
  4. Provide primary care services in a collaborative environment for frail and elderly patients in a sub-acute and long-term care setting.
  5. Pro-actively communicate with care managers, physicians, nursing home staff, and patient families.
  6. Provide primary care that will focus on assessment, health management, education, advocacy and prevention.
  7. Communicate assessment outcomes with patients, physicians and key service providers.
  8. Ensure appropriate utilization and consistent application of the benefits.
  9. Serve as member advocate and facilitator to resolve issues that may be perceived as barriers to care.
  10. Collaborate and communicate with other members of the Care Coordination Team to improve the quality and efficiency of health care delivery.
  11. Rounding on skilled nursing patients as needed.
  12. Thorough and accurate medical documentation, using PCC and other appropriate systems.
  13. Review and order medications, labs and other diagnostic testing.
  14. Participates in Discharge Program and UR meeting.
  15. Attend medical staff meetings.
  16. Document all health assessments, reassessments in the electronic care coordination system and other file systems as appropriate.
  17. Timely completion of all medical records in accordance with facility and other applicable policies.

Qualifications

Position Requirements:

  • Valid Nurse Practitioner state certification/license
  • Strong background in geriatric and/ or internal medicine
  • Experience working with physicians
  • Current CPR certification
  • Demonstrates ability to work independently
